Step 1 – Get in touch with us

Call us on +266 2221 3500 and let us know what you want us to claim.

Step 2 – Find out what form to fill in

There are standard forms that need to be completed and documents that you need to submit to us. We will tell you exactly what we need, and you can simply download the relevant forms.

Complete and sign the Consolidated Provider Plus 10% Risk Premium Checklist.

The following documents must be attached:

• A certified copy of the policyholder's ID /Passport
• A certified copy of proof of premium payment.
• Copy of banking details

Step 3 – Send us the forms

After completing and signing the forms then send them to us along with the relevant documents requested. You can send them to us at this email: myclaim@lnighollard.co.ls.
